Experienced Registered Nurse (Part time) required in Christchurch

HEALTH ADVISOR - Christchurch

Part time - 20 hours per week:- Friday's (4 hours per day), Saturday's and Sunday's (8 hours per day).
Our client is one of the largest providers of support services to people with intellectual and physical disabilities. 

They require a Registered Nurse with experience in the disability sector, community health and primary care, to work across residential and respite services for people with a range of physical and intellectual disabilities. This role is based in Christchurch with travel across the wider city region.

The successful applicant will provide comprehensive and holistic support for people to maximise their wellness and independence, and to advocate for best health outcomes for the people supported including their family/whanau.

We are looking for a dynamic and innovative person who is passionate about delivering high quality individualized support to people with a physical/intellectual disability, whilst working as part of the Health Advisor and wider team.

Role Responsibilities:
  • Review and monitor clients health status and keep accurate/detailed clinical records
  • Ensure client specific health plans and protocols remain up to date
  • Participate in the development and review of holistic wellbeing
  • Provide health related training, coaching and support as required
  • Work in partnership with a multi-disciplinary team of disability/health practitioners and other related professionals.
  • Individual needs assessment, health planning and goal setting
  • Commitment to quality improvement and to work proactively with all key stakeholders, including families/whanau
  • To work flexibly within the Health Advisor Team
  • The ability to participate in an on call roster
